We received your application and regards to the conversation and confirmation with the human resources of your employing company UNIVERSAL HOSPITAL ABUDHABI; based here in UAE concerning your job and the issuance of your working permit/entry visa we confirm further details as follows.
In order to commence the processing of your working and resident permit/entry visa into UAE, you have to complete the copy of the following.
1) Complete filled Visa/work permit Application Form...
2) An address reachable by courier services.
3) Data page of Intl. passport (copy bearing applicant face.)
